Summary

Laser in situ keratomileusis (LASIK) and laser subepithelial keratomileusis (LASEK) offer comparable visual outcomes as a second surgery after initial LASIK failure. While initial recovery may favor LASIK, long-term results show no significant differences in visual acuity or refractive error.

Background:

Unsuccessful laser in situ keratomileusis (LASIK) may require a secondary surgical intervention.
Refractive regression and under-correction are common reasons for repeat laser eye surgery.

Purpose of the Study:

To compare the clinical outcomes of LASIK versus laser subepithelial keratomileusis (LASEK) as a second procedure following an initial failed LASIK surgery.
To evaluate visual acuity, refractive error, corneal curvature, and high-order aberrations after secondary LASIK or LASEK.

Main Methods:

A cohort of 31 patients (62 eyes) underwent a second refractive surgery after prior LASIK.
Eyes were divided into two groups: secondary LASIK (30 eyes) and secondary LASEK (32 eyes).
Clinical parameters were assessed over a one-year follow-up period.

Main Results:

Initial visual acuity at one week postoperation was significantly better in the secondary LASIK group compared to the secondary LASEK group.
No statistically significant differences were observed between the groups in uncorrected visual acuity, spherical equivalent, corneal curvature, or high-order aberrations at one year.
One patient in the LASIK group experienced epithelium ingrowth, and one patient in the LASEK group developed corneal haze.

Conclusions:

Both secondary LASIK and LASEK procedures can achieve satisfactory therapeutic results for patients with prior unsuccessful LASIK.
The choice between secondary LASIK and LASEK may depend on specific patient factors and surgeon preference, considering potential complications like haze or epithelial issues.
