Background:

  • Hyperosmolar nonketotic hyperglycemia (HNH) is a serious diabetic complication.

Observation:

  • A patient presented with an unusual ascending, predominantly motor neuropathy with sensory involvement.
  • Despite polydipsia, the patient did not exhibit expected polyuria due to impaired renal function.

Findings:

  • Insulin therapy resulted in rapid and substantial clinical improvement.
  • The patient was discharged fully mobile and self-sufficient within 15 days.

Implications:

  • The atypical presentation of HNH can mask the underlying metabolic derangement.
  • Early recognition and treatment are crucial for managing HNH and preventing severe neurological deficits.
