Background:

  • Cognitive impairment is a significant issue for aging populations, impacting quality of life and work ability.
  • Maintaining cognitive function in the elderly presents a major challenge for Occupational Medicine.

Purpose of the Study:

  • To investigate the association between intensive endurance exercise and cognitive performance in the elderly.
  • To determine if endurance training increases brain-derived neurotrophic factor (BDNF) and insulin-like growth factor (IGF).

Main Methods:

  • Recruited active elderly marathon runners/cyclists (age > 60) and matched them with inactive controls.
  • Assessed cognitive function using the Vienna Neuropsychological Test Battery and CERAD test battery.
  • Measured levels of BDNF, IGF-1, Apo e4 carrier status, and self-ratings.

Main Results:

  • The endurance exercise group showed better performance in one specific cognitive task (Five Point Test, p=0.04) and a near-significant improvement in another (NAI Stroop Test, p=0.08).
  • No correlation was found between BDNF or IGF-1 levels and exercise duration.
  • Basal levels of BDNF and IGF-1 did not differ between the exercise and control groups.
  • Significantly decreased BDNF levels were observed in individuals with a family history of Alzheimer's disease, despite maintained cognitive performance.

Conclusions:

  • Extensive endurance exercise training may support cognitive function maintenance in older adults.
  • The cognitive benefits of endurance training in this study were not associated with increased BDNF or IGF-1.
  • Reduced BDNF levels in individuals with a family history of Alzheimer's disease suggest a potential precursor role for BDNF reduction in cognitive decline.
