Summary

This study reports the first successful one-stage unifocalization and bidirectional Glenn procedure for a single ventricle with pulmonary atresia and major aortopulmonary collateral arteries (MAPCAs). The novel technique reconstructed the pulmonary artery, yielding excellent long-term results.

Background:

A functionally single ventricle with pulmonary atresia and major aortopulmonary collateral arteries (MAPCAs) presents complex surgical challenges.
Current surgical strategies often involve staged procedures, increasing risks and treatment duration.

Observation:

A novel concomitant one-stage unifocalization and bidirectional Glenn procedure was performed.
Reconstruction of the absent central pulmonary artery utilized MAPCAs and autologous pericardium.

Findings:

Postoperative assessment at 1 year via cineangiography and cardiac catheterization demonstrated excellent outcomes.
Key findings included well-developed pulmonary arteries and normal superior vena cava pressure.
