Summary

Malignant peritoneal mesothelioma, a rare cancer, presents challenges in diagnosis and treatment. Despite poor prognosis, early consideration in ascites cases is crucial for patient management.

Area of Science:

  • Oncology
  • Pathology

Background:

  • Malignant mesothelioma primarily affects the pleura, with infra-diaphragmatic peritoneal mesothelioma accounting for only 10-20% of cases.
  • Pathogenesis is often linked to asbestos and radiation exposure, though idiopathic cases exist.

Observation:

  • Two cases of malignant peritoneal mesothelioma are presented, with one patient lacking a history of known exposures.
  • Diagnosis was confirmed via laparoscopic biopsy and ascites fluid cytology.
  • The condition predominantly affects males over 40 and is often diagnosed at advanced stages.

Findings:

  • Malignant peritoneal mesothelioma has a significantly poorer prognosis (mean survival 5.4 months) compared to pleural mesothelioma (12.5 months).
  • Treatment for localized disease may involve cytoreductive surgery with hyperthermic intraperitoneal chemotherapy.
  • Advanced stages typically require palliative chemotherapy or radiotherapy for symptom management.

Implications:

  • Peritoneal mesothelioma should be included in the differential diagnosis for unexplained ascites.
  • Further research into the pathogenesis and treatment of this rare malignancy is warranted.
  • Improved diagnostic strategies and earlier detection could potentially improve patient outcomes.
