Background:

  • Multiple pulmonary arteriovenous malformations (PAVMs) can lead to significant physiological impairment.
  • Symptoms include orthodeoxia, intrapulmonary shunt, dyspnea, and reduced exercise tolerance.

Observation:

  • A 32-year-old woman with multiple PAVMs presented with severe orthodeoxia, intrapulmonary shunt, dyspnea, and limited exercise tolerance.
  • She underwent balloon and coil embolization targeting her three largest PAVMs.

Findings:

  • Post-embolization, the patient exhibited marked improvement in symptoms, orthodeoxia, and intrapulmonary shunt.
  • Objective measures showed sustained enhancement in oxygenation, reduced P(A-a)O2 difference, and increased maximum oxygen consumption during exercise.
