Summary

The Harmonic Scalpel (HS) offers superior efficacy for cystic duct closure in laparoscopic cholecystectomy compared to the PlasmaKinetic Sealer (PK). HS demonstrated comparable safety and effectiveness to surgical clips (SC).

Background:

Limited data exists on Harmonic Scalpel (HS) efficacy for cystic duct ligation.
No literature is available on PlasmaKinetic Sealer (PK) use for cystic duct closure in laparoscopic cholecystectomy (LC).

Purpose of the Study:

To compare the efficacy and safety of HS and PK for cystic duct closure during LC.
To evaluate the performance of HS and PK against standard surgical clips (SC).

Main Methods:

Prospective case-control study involving 90 patients undergoing LC.
Cystic ducts were sealed with SC, HS, or PK.
Bursting pressures of sealed cystic ducts were measured using saline infusion.

Mean cystic duct bursting pressures: SC (332.46 ± 4.62 mmHg), PK (326.56 ± 4.53 mmHg), HS (343.06 ± 4.28 mmHg).
HS showed superior results in terms of bursting pressure.

Conclusions:

Harmonic Scalpel (HS) is more effective than PlasmaKinetic Sealer (PK) for cystic duct closure.
HS is as safe and effective as surgical clips (SC) for this procedure.
