    Clinically undiagnosed pulp microexposures can occur during deep cavity preparations. Approximately one-fifth of class II preparations with less than 0.5 mm remaining dental thickness may have these small pulp exposures.

    Background:

    • Pulp exposure during cavity preparation can compromise tooth vitality.

    Purpose of the Study:

    • To investigate the probability of clinically undiagnosed pulp microexposures in deep class II preparations.
    • To determine the frequency of pulpal microexposures in relation to remaining dental thickness.

    Main Methods:

    • Thirty deep class II preparations in human premolars were examined.
    • The axial wall of each preparation was analyzed for microexposures post-extraction.
    • Remaining dental thickness (RDT) was a key parameter assessed.

    Main Results:

    • Seven out of 30 teeth (23.7%) exhibited some form of pulp exposure.
    • A significant correlation between RDT and microexposure was suggested.
    • The study identified specific risk factors for pulpal insults.

    Conclusions:

    • Approximately one-fifth of class II preparations with RDT < 0.5 mm may present with pulpal microexposures.
    • Clinicians should be aware of the potential for undiagnosed microexposures in deep preparations.
