Background:

  • Taiwan is an endemic region for viral hepatitis A, primarily transmitted via the fecal-oral route.
  • A nationwide policy mandating disposal tableware in public eating places was implemented in 1982 to curb transmission.
  • This study aimed to assess Hepatitis A seroprevalence in Taiwanese workers and evaluate policy impact.

Purpose of the Study:

  • To estimate Hepatitis A seroprevalence among industrial park workers in Taiwan in 2005.
  • To identify risk factors associated with Hepatitis A infection.
  • To evaluate the long-term effectiveness of the disposal tableware policy on Hepatitis A transmission.

Main Methods:

  • Recruitment of 11,777 workers during annual health examinations in 2005.
  • Measurement of anti-hepatitis A virus IgG titers using microparticle enzyme immunoassay.
  • Comparison of seroprevalence across birth cohorts and analysis of historical study data.

  • An overall sero-positive rate of 22.0% was observed.
  • Significantly lower seroprevalence in individuals born after 1982 (2.7%) compared to those born before (25.3%).
  • Age-specific seroprevalence decreased with calendar year for pre-1982 cohorts and stabilized for post-1982 cohorts.

Conclusions:

  • The findings strongly support the effectiveness of disposal tableware in preventing Hepatitis A transmission.
  • The policy demonstrated a significant reduction in Hepatitis A seroprevalence, particularly for cohorts born after its implementation.
  • This study provides evidence for the successful public health intervention in controlling viral hepatitis A.
