Context:

  • Current gynecologic cancer staging systems require refinement due to advances in oncology.

Purpose:

  • To revise and improve the International Federation of Gynecology and Obstetrics (FIGO) staging system for gynecologic cancers.
  • To enhance prognostic accuracy and treatment planning for patients with gynecologic neoplasms.
  • To establish a standardized staging system for sarcomas based on soft tissue sarcoma criteria.

Summary:

  • Revisions to vulvar cancer staging involve combining Stages I and II, focusing on node number, morphology, and discounting bilaterality.
  • Cervical cancer staging sees the deletion of Stage 0 and inclusion of substages in Stage IIA.
  • Endometrial cancer staging combines IA and IB, reclassifies endocervical glandular involvement as Stage I, and separates pelvic/para-aortic node involvement. A new sarcoma staging system was also established.
