Background:

Purpose of the Study:

  • To describe the spectrum of pathologies in patients with unilateral sinonasal symptoms.
  • To identify predictors of neoplastic pathology.

Main Methods:

  • Retrospective chart review of 207 patients with sinonasal symptoms.
  • Analysis of 63 patients with unilateral symptoms and CT scan opacification.
  • Evaluation of presenting symptoms, radiographic, surgical, and pathologic findings.

Main Results:

  • A diverse range of pathologies were identified, including polyps, sinusitis, cysts, and 13 neoplastic diseases (benign and malignant).
  • Rare neoplastic conditions included pleomorphic adenoma, ameloblastoma, schwannoma, myxoma, and non-Hodgkin lymphoma.
  • Nasal discharge was more frequent in inflammatory disease, whereas epistaxis was more common in neoplastic disease.

Conclusions:

  • Unilateral sinonasal symptoms encompass a broad differential diagnosis.
  • Clinical history, examination, and imaging aid diagnosis, but histologic confirmation is essential.
  • Rare neoplastic entities should be considered in the differential diagnosis of unilateral sinonasal masses.
