Microbiome of the Eye01:22

Microbiome of the Eye

The human eye has a specialized microbiota that reflects its unique anatomical and immunological environment. This low-biomass microbial community predominantly colonizes the conjunctiva and eyelid margins, playing a vital role in ocular surface homeostasis and defense. Despite its proximity to the richly colonized facial skin, the ocular surface maintains a distinct microbial profile due to continuous mechanical and biochemical defense mechanisms. The conjunctival surface hosts fewer microbial...

Contact lens wearers with keratitis show specific germ spectra, with higher detection rates in lens and container swabs compared to conjunctival swabs. Combination therapy with gentamicin and ofloxacin showed better visual outcomes than newer agents.

Area of Science:

  • Ophthalmology
  • Microbiology
  • Infectious Diseases

Background:

  • Contact lens (CL) wear affects over 125 million people globally.
  • CL-induced keratitis occurs in 0.011-0.2% of wearers annually.
  • Optimizing antibiotic therapy requires understanding the spectrum of causative organisms.

Purpose of the Study:

  • To analyze the spectrum of germs in contact lens-induced keratitis.
  • To compare microbiological analysis techniques for optimizing antibiotic therapy.
  • To evaluate visual acuity outcomes with different antibiotic regimens.

Main Methods:

  • Retrospective study of 65 eyes with CL-induced keratitis or corneal ulceration (2005-2010).
  • Comparison of best-corrected visual acuity (BCVA) changes under single and combined antibiotic therapies.

Conjunctival Commensal Isolation and Identification in Mice

Published on: May 1, 2021

  • Microbiological analysis of conjunctival swabs (CS) and contact lens/container swabs (CLS).
  • Main Results:

    • Soft contact lens wearers constituted 96.9% of cases.
    • Microbiological findings were more frequent in CLS (81.3%) than CS (48.8%).
    • Coagulase-negative Staphylococcus and Pseudomonas species were common pathogens.
    • Gram-negative bacteria predominated in CLS, while Gram-positive bacteria were more common in CS.
    • No significant difference in BCVA change between single and double antibiotic therapy.
    • Gentamicin & ofloxacin combination showed better BCVA outcomes than moxifloxacin & tobramycin.
    • BCVA change did not differ significantly between patients with positive or negative CS results.

    Conclusions:

    • Soft lens wear is strongly associated with specific microbial keratitis spectra.
    • Conjunctival swab findings are less critical than lens/container swabs due to lower detection rates and comparable outcomes.
    • Single vs. dual antibiotic therapy showed no significant difference in BCVA development.
    • Ofloxacin with gentamicin demonstrated superior BCVA outcomes compared to newer agents.
    • Increased microbiological analysis of contact lenses and their containers is recommended for improved treatment strategies.
