Observation:

  • A 47-year-old male with advanced gastric cancer presented with obstructive jaundice and was a candidate for surgical bilioenteric anastomosis.
  • Intraoperative findings revealed a distal choledochal obstruction and mesenteric retraction due to a lymph node mass, precluding standard jejunal bypass.
  • An alternative bilioenteric bypass using an ileal loop interposition was successfully performed.

Findings:

  • The patient experienced an uneventful recovery with normalized bilirubin levels within one week post-surgery.
  • The ileal loop interposition effectively restored bile flow, resolving the obstructive jaundice.

Implications:

  • This alternative biliary bypass technique is safe, easily performed, and provides a viable option for select surgical candidates.
  • It is particularly beneficial for patients with better life expectancy and when anatomical challenges prevent standard jejunal anastomosis.
  • This approach expands palliative surgical options for managing obstructive jaundice in advanced gastrointestinal malignancies.
