Background:

Self-injurious behavior (SIB) in older adults is harm inflicted without suicidal intent, posing clinical challenges.
Understanding SIB as distinct from suicidal behavior is limited.
SIB causes significant concern for families and caregivers of older adults.

Purpose of the Study:

To review the existing literature on self-injurious behavior in the geriatric population.
To identify the prevalence and characteristics of SIB in older adults, especially in nursing home settings.

Main Methods:

A systematic literature search was conducted in PubMed, Ovid Medline, and ScienceDirect from 1970 to 2009.
Keywords included "self-injurious behavior," "older adults," and "nursing homes."
Studies focusing on "self-harm behavior" with suicidal intent were excluded to isolate SIB.

Main Results:

Literature on SIB in older adult nursing home residents is sparse.
SIB prevalence in nursing home residents aged 65+ was reported as high as 14% in one study.
SIB is associated with dementia, poor impulse control, physical isolation, and risk of accidental death.

Conclusions:

Self-injurious behavior is likely prevalent among older adults in nursing homes.
There is a significant lack of evidence-based treatment guidelines for SIB in the elderly population.
