Background:

Sarcoidosis is a multisystem inflammatory disease characterized by non-caseating granulomas.
Cutaneous involvement is common in sarcoidosis, presenting in various forms.
Tattoo reactions can sometimes mimic or be associated with underlying systemic conditions.

Observation:

A 41-year-old male presented with a six-month history of bumps in black tattoos on his trunk and arms.
He subsequently developed eye pain diagnosed as uveitis, followed by rough skin patches and hair thinning.
Clinical examination revealed widespread eruptions consistent with sarcoidosis.

Skin biopsy confirmed sarcoidal granulomas within tattoos and perifollicular areas.
