Summary

Diffusion tensor imaging (DTI) can detect upper motor neuron degeneration in Primary Lateral Sclerosis (PLS). This study found decreased fractional anisotropy in the pyramidal tract, indicating nerve cell damage.

Background:

  • Primary Lateral Sclerosis (PLS) is a rare neurodegenerative disorder impacting upper motor neurons.
  • Diagnosis of PLS is primarily clinical.
  • Diffusion Tensor Imaging (DTI) offers quantitative assessment of white matter integrity.

Observation:

  • A single patient diagnosed with PLS was compared to eight age-matched healthy controls.
  • DTI was employed to analyze fractional anisotropy (FA) on a voxel-by-voxel basis.

Findings:

  • The study observed reduced FA in the bilateral proximal pyramidal tracts.
  • This finding suggests degeneration of the pyramidal cells, consistent with upper motor neuron involvement.

Implications:

  • Voxel-based DTI shows potential as an objective biomarker for upper motor neuron degeneration in PLS.
  • This technique could aid in the diagnosis and monitoring of PLS progression.
