Area of Science:

  • Cardiovascular Surgery
  • Hypothermic Management
  • Aortic Arch Replacement

Background:

  • Profound hypothermic circulatory arrest (HCA) poses significant risks during aortic arch replacement.
  • Maintaining adequate cerebral perfusion is critical during complex aortic surgeries.

Observation:

  • A novel strategy utilizing moderate hypothermia was implemented for aortic arch replacement.
  • A three-pump system was employed for targeted organ perfusion: brain, thoracoabdominal aorta, and heart.

Findings:

  • Two patients underwent successful complete aortic arch replacement using the moderate hypothermia strategy.
  • No complications were observed, and both patients were extubated without issue.
  • The method effectively preserved cerebral blood flow autoregulation, avoiding high vascular resistances.

Implications:

  • This approach presents a potentially safer alternative to profound HCA for aortic arch surgery.
  • The technique may reduce the incidence of neurological and systemic complications.
  • Further research could validate this method for broader clinical application in aortic surgery.
