Background:

  • Mooren's ulcer is a severe form of peripheral ulcerative keratitis.
  • Treatment options for Mooren's ulcer can be limited, especially in cases resistant to medical therapy or conjunctival resection.

Purpose of the Study:

  • To evaluate the efficacy of amniotic membrane transplantation in treating Mooren's ulcer.
  • To assess the outcomes of epithelial healing and visual acuity following the procedure.

Main Methods:

  • A prospective interventional case series involving 18 eyes of 14 patients with Mooren's ulcer.
  • Amniotic membrane grafts (single or multilayer) were applied to the ulcerated corneas.
  • Surgical techniques included overlay and freehand grafts tailored to the defect.

Main Results:

  • Successful epithelial healing was achieved in 16 out of 18 eyes after a single surgery, with a mean healing time of 12.4 days.
  • Two eyes required a second graft due to complications (subgraft hemorrhage, persistent epithelial defect).
  • Visual acuity stabilized in all eyes, with 10 eyes achieving 6/12 or better vision.

Conclusions:

  • Amniotic membrane transplantation is a potentially useful surgical treatment for selected Mooren's ulcer cases.
  • This method is particularly valuable when systemic immunosuppressive drugs are inaccessible or contraindicated.
