Purpose of the Study:

  • To report the long-term results of the original cementless dual mobility socket (Series A) and midterm results of the second generation (Series B).
  • To evaluate the survival rates and reasons for revision in both series.

Main Methods:

  • Retrospective analysis of 437 primary cementless total hip arthroplasties (Series A) with a mean follow-up of 16.5 years.
  • Retrospective analysis of 231 primary cementless total hip arthroplasties (Series B) with a mean follow-up of 5 years.
  • Comparison of revision rates for aseptic loosening and dislocation between the two generations of dual mobility sockets.

Main Results:

  • Series A (original socket) showed a 15-year survival rate of 84.4% with 6.8% revision for aseptic loosening and 5 revisions for dislocation.
  • Series B (second generation socket) demonstrated a 5-year survival rate of 99.6% with no observed dislocations or mechanical revisions.
  • The second-generation dual mobility socket significantly reduced revision rates for dislocation and mechanical failure.

Conclusions:

  • Cementless dual mobility sockets offer long-term stability, particularly for patients over 70 or those at high risk of dislocation.
  • While dual mobility concepts do not eliminate wear and aseptic loosening, they are effective in preventing dislocation.
  • The second-generation dual mobility socket shows promising midterm results with excellent stability and low revision rates.
