Background:

  • Permanent hypoparathyroidism is a severe complication following thyroid surgery.
  • Some patients exhibit persistent hypocalcemia and symptoms despite normal parathyroid hormone (PTH) levels post-thyroidectomy.

Purpose of the Study:

  • To evaluate parathyroid function impairment after thyroidectomy in patients with long-term hypocalcemia and normal PTH.
  • To determine associations between these metabolic disturbances and bone/calcium metabolism.

Main Methods:

  • A series of eight patients with normal PTH and subnormal calcium levels 2 months post-thyroid surgery were studied.
  • Intraoperative and long-term follow-up assessed PTH/calcium kinetics and various bone/calcium metabolism markers.

Main Results:

  • All eight patients presented with hypocalcemia and normal PTH levels during long-term follow-up (13.8 ± 2.4 months post-surgery).
  • Significant intraoperative PTH decline (>90%) was observed, reaching a trough 3 hours after surgery.
  • Isolated abnormalities in bone and calcium metabolism were noted in five patients.

Conclusions:

  • Intraoperative parathyroid gland or vascular injury likely causes permanent hypocalcemia with normal PTH post-thyroid surgery.
  • The remaining parathyroid tissue maintains normal PTH levels, but these are insufficient to normalize serum calcium.
  • The term "parathyroid insufficiency" may be more appropriate than "hypoparathyroidism" for these patients.
