During in vitro fertilization (IVF), rising estradiol levels increase clotting factors like von Willebrand factor (VWF) and decrease ADAMTS13, a VWF-regulating enzyme. This shift in hemostasis may explain the heightened thrombotic risk in IVF patients.
Area of Science:
- Reproductive Endocrinology
- Hematology
- Thrombosis Research
Background:
- Elevated estradiol during IVF can impact hemostasis.
- High von Willebrand factor (VWF) levels are a risk factor for thrombosis.
- ADAMTS13, the VWF-cleaving protease, plays a key regulatory role but its changes during IVF are unknown.
Purpose of the Study:
- To investigate the impact of IVF-induced estradiol fluctuations on VWF and ADAMTS13 levels.
- To explore the relationship between these hemostatic factors and thrombotic risk in IVF patients.
Main Methods:
- Blood samples were collected from 31 women undergoing IVF.
- Measurements included estradiol, VWF antigen and activity, factor VIII, and ADAMTS13 antigen and activity.
- Samples were taken during maximal estradiol downregulation and high estradiol stimulation phases.
Main Results:
- Estradiol levels significantly increased from 154 pg/ml to 5889 pg/ml.
- Factor VIII, VWF antigen, and VWF activity all showed significant increases (P < 0.001).
- ADAMTS13 antigen and activity significantly decreased (P < 0.05 and P < 0.01, respectively).
Conclusions:
- IVF treatment leads to increased VWF and factor VIII, alongside decreased ADAMTS13.
- These hemostatic changes during IVF may contribute to an elevated risk of thrombotic events.
