Adult intussusception is rare and often presents as acute bowel obstruction. Surgical resection is effective, with no recurrence observed in the resection group, suggesting optimal outcomes.
Area of Science:
- Gastroenterology
- Surgical Oncology
Background:
- Intussusception, a rare condition in adults, typically presents as acute intestinal obstruction.
- Understanding its clinical presentation and treatment outcomes is crucial for effective management.
Purpose of the Study:
- To determine the clinical presentation and treatment outcomes of adult patients diagnosed with intussusception.
- To evaluate the role of diagnostic tools and surgical interventions.
Main Methods:
- A case series study was conducted at Aga Khan University Hospital, Karachi, from January 1988 to December 2008.
- Medical records of adult patients (age >15) with intussusception were reviewed, excluding those with incomplete data or less than 30 days follow-up.
- Data analysis included hospital stay, 30-day morbidity, mortality, and recurrence rates.
Main Results:
- Nineteen adult patients (mean age 37 years, male predominance) presented with acute bowel obstruction, predominantly small bowel intussusception (14 patients).
- CT scan diagnosed intussusception in 10 of 12 patients. Benign lead points were identified in 80% of cases.
- Eighteen patients underwent surgery; 14 had resection with primary anastomosis, and 4 had reduction only. No recurrence was seen in the resection group, while one recurrence occurred in the reduction group. One death resulted from advanced gastrointestinal malignancy.
Conclusions:
- Intussusception is an uncommon cause of acute intestinal obstruction in adults.
- CT scan is a valuable tool for pre-operative diagnosis.
- Early surgical resection, with or without reduction based on bowel viability, leads to optimal outcomes, and benign lead points are common.
