Background:

  • Orthognathic surgery aims to correct jaw and facial irregularities.
  • Neurologic complications, such as altered sensation, can occur post-surgery.
  • Understanding the natural recovery of these neurologic injuries is crucial for patient management.

Purpose of the Study:

  • To evaluate the natural recovery of neurologic injury after orthognathic surgery.
  • To assess subjective neurologic changes using patient-reported outcomes.
  • To analyze the impact of specific surgical procedures, including genioplasty, on sensory recovery.

Main Methods:

  • A cohort of 47 patients undergoing various orthognathic procedures (BSSO, genioplasty, Le Fort I) were evaluated.
  • Subjective neurologic assessments were conducted at 1, 3, and 6 months post-surgery.
  • Visual Analog Scale (VAS) was used to quantify pain and altered sensation.

Main Results:

  • Sensory alterations were most frequently reported in the chin (55.7%) and lip (27.3%).
  • VAS scores for altered sensation significantly decreased over the 6-month follow-up period.
  • While altered sensation was common after genioplasty, it did not correlate with patient age or concurrent maxillary surgery.

Conclusions:

  • Altered sensation is a frequent, though often unavoidable, complication of orthognathic surgery.
  • Spontaneous resolution of sensory disturbances is possible over time.
  • Simultaneous genioplasty increases the incidence of altered sensation but does not appear to affect long-term outcomes significantly in relation to age or maxillary surgery.
