Summary

Allergic fungal sinusitis can cause optic nerve compression and vision loss. Urgent surgery to remove fungal debris and decompress the optic nerve can restore vision in partial cases.

Background:

  • Allergic fungal sinusitis (AFS) is a non-invasive fungal infection of the paranasal sinuses.
  • AFS can lead to orbital and optic nerve complications due to mass effect and inflammation.
  • Optic nerve involvement in AFS can result in significant visual impairment.

Purpose of the Study:

  • To characterize the clinical presentation and outcomes of optic nerve involvement in patients with AFS.
  • To evaluate the efficacy of surgical intervention for visual recovery in these cases.

Main Methods:

  • Retrospective analysis of prospectively collected data from 10 consecutive AFS patients with visual loss.
  • Total of 70 AFS cases undergoing surgery between June 1997 and May 2007 were reviewed.
  • 13 affected eyes from 10 patients with visual loss due to optic nerve compression were analyzed.

Main Results:

  • Mean duration of rhinological symptoms before visual loss was 22 months.
  • Visual acuity at presentation ranged from 6/12 to no light perception; 3 of 10 cases had bilateral involvement.
  • Urgent surgical decompression led to visual recovery in 7 of 13 eyes; recovery was less likely with long-standing or complete visual loss.

Conclusions:

  • Chronic allergic fungal sinusitis can lead to optic nerve compression and subsequent vision loss.
  • Prompt surgical decompression and removal of fungal material are crucial for visual recovery.
  • Early intervention improves outcomes, particularly in cases with partial visual impairment.
