In aging men, common symptoms like low libido and erectile dysfunction are linked to age, not just testosterone levels. Defining specific testosterone thresholds for these symptoms proved difficult due to age being a significant confounding factor.

Background:

  • Androgen deficiency symptoms are common in aging males.
  • Previous studies have suggested associations between testosterone levels and these symptoms.
  • However, the role of age as a confounding factor requires further investigation.

Purpose of the Study:

  • To investigate the association between specific clinical symptoms (e.g., low libido, erectile dysfunction) and testosterone levels in aging men.
  • To determine if symptom-specific testosterone thresholds can be defined.
  • To assess the confounding effect of age on these associations.

Main Methods:

  • Serum testosterone levels were measured in 675 healthy men.
  • Participants completed validated questionnaires including the Aging Males Symptoms scale, Beck Depression Index, and International Index of Erectile Function.
  • Statistical analyses, including univariate and multiple multinomial regression with Bonferroni correction, were used to evaluate symptom-testosterone and symptom-age associations.

Main Results:

  • Significant associations between testosterone levels and androgen deficiency symptoms were observed at 13.5-14.4 nmol/L, but age was a confounding factor.
  • Symptoms like loss of libido, lack of vigour, and sexual dysfunction were primarily associated with age, not testosterone levels.
  • Erectile dysfunction showed a weak association with testosterone levels (14.65-14.8 nmol/L) but was significantly linked to age.

Conclusions:

  • Androgen deficiency symptoms in aging males can occur at normal testosterone levels.
  • Age is a significant confounder when evaluating the relationship between testosterone and these symptoms.
  • Symptom-specific testosterone thresholds could not be definitively established in this study.
