Tumor Progression

Tumor progression is a phenomenon where the pre-formed tumor acquires successive mutations to become clinically more aggressive and malignant. In the 1950s, Foulds first described the stepwise progression of cancer cells through successive stages.

  • Cholangiocarcinoma (CCA) is a rare biliary epithelium cancer with a poor prognosis.
  • CCA can originate in the intrahepatic, hilar, or extrahepatic bile ducts.

Purpose of the Study:

  • To review the principles and current trends in cholangiocarcinoma treatment.
  • To discuss diagnostic tools and prognostic factors for CCA.

Main Methods:

  • Systematic literature search of PubMed/MEDLINE (1989-2008).
  • Focus on English-language articles related to "cholangiocarcinoma" and "treatment".

Main Results:

  • Preoperative diagnostic tools include ERCP, digital image analysis, FISH, and MRCP.
  • Surgical management is the only curative option for cholangiocarcinoma.
  • Many patients present with unresectable tumors, leading to a poor prognosis.

Conclusions:

  • Aggressive surgical management in selected cholangiocarcinoma cases can lead to prolonged survival.
  • Despite significant morbidity and mortality, surgery offers the best outcome for resectable CCA.
