Background:

  • Neurofibromatosis Type 1 (NF1) is a genetic disorder with diverse clinical manifestations.
  • Diploic hematomas are rare intracranial lesions typically associated with head trauma.

Observation:

  • A 23-year-old woman with NF1 stigmata presented with a spontaneous diploic hematoma of the left parietal bone.
  • Radiography revealed an osteolytic lesion with abnormal vascular markings.
  • Angiography showed an incidental carotid artery aneurysm and unusual venous drainage into diploic vessels.

Findings:

  • This is the first reported case of a non-traumatic diploic hematoma in a patient with NF1.
  • Abnormal venous drainage into the diploic space and vasculopathy causing venous obstruction are hypothesized as causative factors.

Implications:

  • This case expands the understanding of potential vascular complications in neurofibromatosis Type 1.
  • Highlights the importance of considering vascular abnormalities in NF1 patients presenting with intracranial lesions.
