  • Itching and papules are typical symptoms, but atypical presentations can occur.

Observation:

  • A 79-year-old male presented with a bullous eruption mimicking autoimmune blistering disease.
  • Initial investigations, including immunofluorescence and autoantibody testing, were negative.
  • Skin biopsy unexpectedly revealed scabies mites.

Findings:

  • Histological examination confirmed scabies infestation as the cause of the bullous eruption.
  • The patient responded well to treatment with 20% benzyl benzoate.
  • This case highlights a rare presentation of scabies.

Implications:

  • Bullous scabies should be considered in the differential diagnosis of bullous eruptions, especially when steroid-resistant and immunopathologically negative.
  • Accurate diagnosis requires a high index of suspicion and potentially skin biopsy.
  • Prompt identification and treatment are essential to prevent complications and transmission.
