Background:

  • Soluble TNF-alpha receptors (sTNF-R1, sTNF-R2) are key inflammatory markers.

Purpose of the Study:

  • To investigate the relationship between circulating sTNF-R1 and sTNF-R2 levels and respiratory function in obese individuals.

Main Methods:

  • Prospective study of 31 non-diabetic, non-smoking, morbidly obese women.
  • Pulmonary function tests (spirometry, lung volumes, polygraphy, blood gases) were performed.
  • ELISA was used to measure sTNF-R1, sTNF-R2, IL-6, and adiponectin levels.

Main Results:

  • sTNF-R1 positively correlated with BMI and PaCO2, and negatively with FEV1, FEF(25-75), and FVC.
  • sTNF-R1 independently predicted reduced FEV1 and FVC.
  • No significant correlation was found between sTNF-R2 and respiratory parameters.

Conclusions:

  • Elevated sTNF-R1 levels are associated with impaired lung volumes and airflow limitation in morbidly obese patients.
  • sTNF-R1 may serve as an early indicator of respiratory compromise in obesity.
  • Further research on anti-TNF-alpha agents for pulmonary function in obesity is warranted.
