Enzyme-Linked Immunosorbent Assay01:33

Enzyme-Linked Immunosorbent Assay

14.2K
In 1971, Peter Perlman and Eva Engvall developed an Enzyme-linked immunosorbent assay (ELISA or EIA). ELISA differs from western blot in that the assays are conducted in microtiter plates or in vivo rather than on an absorbent membrane.
There are many different types of ELISAs, but they all involve an antibody molecule whose constant region binds an enzyme, leaving the variable region free to bind its specific antigen.  Enzyme-substrate reaction allows the antigen to be visualized or...

Immunodeficiency Diseases01:25

Immunodeficiency Diseases

3.8K
Immunodeficiency disorders are conditions in which the immune system's ability to fight infectious disease and cancer is compromised or entirely absent. The immune system comprises a complex network of cells, tissues, and organs that work together to protect the body from potentially harmful invaders. When this system is deficient or not functioning properly, it leaves the body susceptible to infections, diseases, or other complications.
There are three main causes of immunodeficiency...

Drugs for Treatment of Crohn's Disease in IBD Using Immunomodulatory Agents

780
Crohn's disease is an inflammatory bowel disorder marked by chronic inflammation of the GI tract. Various treatment strategies for Crohn's disease are employed, such as immunomodulatory agents, glucocorticoids, and biologics or anti-TNF therapy. Azathioprine (Imuran), a commonly used immunomodulatory drug for Crohn's disease, is converted in the body to mercaptopurine, which inhibits purine biosynthesis and cell proliferation. Both are utilized in severe cases of Inflammatory Bowel...

Hypersensitivity Reactions: Immune-Complex Reactions

287
Type III hypersensitivity reactions occur when antigen–antibody complexes form and activate the complement system. Normally, these complexes help the clearance of antigens by phagocytes and red blood cells. However, when large numbers of immune complexes are present, they can deposit in tissues—particularly in the walls of blood vessels—leading to inflammation and tissue injury. These deposits trigger complement activation and neutrophil recruitment, resulting in serum...

    Summary

Immunoadsorption (IAS) offers rapid removal of pathogenic factors for severe systemic lupus erythematosus (SLE) patients. While experimental and costly, IAS is a vital rescue therapy for life-threatening conditions due to its fast action and safety.

    Area of Science:

    • Immunology
    • Nephrology
    • Extracorporeal Therapies

    Background:

    • Immunoadsorption (IAS) serves as a rescue therapy for severe systemic lupus erythematosus (SLE) patients unresponsive to standard treatments.
    • This extracorporeal technique efficiently removes pathogenic immunocomplexes (ICs) and autoantibodies (Abs).

    Discussion:

    • IAS demonstrates short- to mid-term efficacy and biocompatibility in SLE, particularly for renal involvement.
    • Despite lacking randomized controlled trial (RCT) evidence, IAS is employed in critical situations.
    • The procedure is considered experimental and relatively expensive.

    Key Insights:

    • IAS provides rapid removal of disease-causing molecules in severe SLE.
    • The therapy has an acceptable safety profile for critically ill patients.

  • Efficacy and biocompatibility are supported by existing data, though RCT evidence is pending.
  • Outlook:

    • Further research, including RCTs, is needed to solidify the role of IAS in SLE management.
    • Exploring cost-effectiveness and wider accessibility of IAS could enhance its clinical utility.
    • IAS may become a more established treatment option for refractory SLE cases with more robust evidence.
