Percutaneous aortic valve implantation offers a safe alternative for severe aortic stenosis patients ineligible for surgery. This transcatheter procedure improved quality of life, demonstrating its therapeutic potential.

Area of Science:

  • Cardiology
  • Interventional Cardiology
  • Medical Devices

Background:

  • Severe aortic stenosis poses significant risks for traditional valve replacement surgery.
  • Percutaneous aortic valve implantation (PAVI) is an emerging minimally invasive treatment option.
  • PAVI is indicated for high-risk patients previously denied surgical valve replacement.

Purpose of the Study:

  • To evaluate the safety and efficacy of PAVI in high-risk patients.
  • To assess the procedural success and clinical outcomes of PAVI.
  • To determine the long-term impact of PAVI on patient quality of life.

Main Methods:

  • PAVI was performed using a self-expanding bioprosthesis delivered via an 18F catheter.
  • Procedures were primarily conducted via femoral artery access under local anesthesia and sedation.

  • Thirty-two patients (mean age 80 years, logistic EuroSCORE 22%) were treated.
  • Main Results:

    • Successful implantation in all patients with significant reduction in transvalvular gradients.
    • Procedure-related mortality occurred in two patients; common complications included vascular access issues and atrioventricular block.
    • Improved functional capacity observed post-procedure, with no prosthesis-related events at mean 13-month follow-up.

    Conclusions:

    • PAVI is a safe and effective treatment for severe symptomatic aortic stenosis in high-risk patients.
    • The procedure leads to improved quality of life and functional capacity.
    • PAVI represents a viable alternative to surgical aortic valve replacement for select patient populations.
