Observation:

  • A 22-year-old male presented with severe trismus, pain, and temporal swelling post-mandibular third molar extraction.
  • CT scan revealed an inflammatory process within the temporal space.
  • Initial intravenous cephalosporin treatment was insufficient, necessitating further intervention.

Findings:

  • Incision and drainage from both extraoral and intraoral approaches were performed.
  • The patient's antibiotic regimen was adjusted to clindamycin post-discharge.
  • This case underscores the potential for mandibular third molar infections to spread to the temporal space.

Implications:

  • While retromaxillary and temporal infections are common after maxillary molar extractions, their occurrence after mandibular third molar extraction is less frequent.
  • The spread mechanism and microbial virulence likely play a role in the ascension of infection.
  • Further research is needed to fully understand the pathogenesis of these rare but severe infections.
