Purpose of the Study:

  • To investigate the primary causes of wide fluctuations in human chorionic gonadotropin (hCG) concentrations during early gestation.
  • To compare the impact of different gestational dating methods on observed hCG variability.

Main Methods:

  • Daily urine samples were collected from 82 women during the first 6 weeks of gestation.
  • Luteinizing hormone (LH) and hCG levels were monitored in first-void urine samples.
  • Pregnancy dating was anchored to the last menstrual period, LH peak, and the day of implantation for comparative analysis.

Main Results:

  • Anchoring pregnancy to the last menstrual period or LH peak resulted in wide hCG variations (677 ± 786-fold and 810 ± 936-fold, respectively).
  • Anchoring pregnancy to the day of implantation significantly reduced hCG variation (187 ± 123-fold, P < .00005).
  • Individual differences in daily hCG production rates (ranging from 1.52-fold to 2.92-fold) were identified as a major contributor to overall hCG variation.

Conclusions:

  • Wide fluctuations in early pregnancy hCG levels are attributed to two main factors.
  • Inaccurate pregnancy dating based on the last menstrual period, rather than implantation timing, significantly increases perceived hCG variation.
  • Individual variations in the rate of hCG production also contribute substantially to the observed fluctuations in hormone concentrations.
