Background:

Orofacial granulomatosis (OFG) is an uncommon but increasingly recognized inflammatory disorder.
It leads to significant patient morbidity, including persistent and painful oral ulcerations and soft tissue enlargement.

Observation:

A female patient presented with persistent, undiagnosed upper lip enlargement.
Previous consultations with general dental practitioners and physicians failed to establish a diagnosis.

Findings:

Ultrasonography served as an effective adjunctive tool in diagnosing orofacial granulomatosis.
Treatment with a combination of topical and intra-lesional steroids resulted in successful management.
