Ethical Dilemmas II01:30

Ethical Dilemmas II

Resolving an ethical dilemma in healthcare involves a systematic approach that considers every aspect of the issue, respecting both the patient's needs and values and the healthcare professional's ethical obligations. Here are potential steps to resolve an ethical dilemma:
Ethical Issues01:27

Ethical Issues

Nurses are essential in patient care, upholding the ethical principles of their profession and effectively navigating ethical dilemmas. Neglecting ethical issues can lead to inadequate patient care, compromised therapeutic relationships, and moral distress among healthcare workers.

Patient-centered Care01:13

Patient-centered Care

Patient-centered care involves delivering care beyond inpatient hospitalization. Reflective practice can enhance a patient-centered approach. Reflective practice is a process of reasoning that considers all aspects of the present situation, including practicalities, learning from personal practice, and consideration of patient needs. Patients appreciate care decisions made while considering their input. Involving the patient in their care provides the patient with a sense of contribution rather...
Barriers to Effective Communication II01:21

Barriers to Effective Communication II

The barriers to effective communication also include cultural barriers, semantic barriers, gender barriers, and time constraints.
Cultural barriers:
Differences in values, beliefs, religion, knowledge, and tradition can significantly impact communication. Awareness of nonverbal cues is critical, especially when conversing with a patient from a different culture. What appears appropriate in one culture may be inappropriate in another.
Semantic barriers:
As a result of their tendency to use...

Interdisciplinary Care: The Health Care Team-II

An interdisciplinary team includes many healthcare professionals working together and utilizing their skills, knowledge, and expertise to provide holistic and quality patient care. Here are a few more healthcare professionals.

Obedience01:08

Obedience

According to obedience research, we may harm others under the forceful pressures of an authority figure (Milgram, 1974). How about if the inappropriate orders were delivered with less force? The increasing interdependence between nurses and physicians compelled Hofling and his colleagues to explore nurses’ reactions to a potentially harmful medical request made by the perceived authority figure, the doctor (Hofling, Brotzman, Dalrymple, Graves, & Pierce, 1966). In this situation, obedience...

Summary

Difficult medical encounters, often involving patients with unexplained symptoms, leave both patients and physicians unsatisfied. Enhanced communication and psychosocial training may reduce these challenging patient-physician interactions.

Area of Science:

  • Internal Medicine
  • Psychosomatic Medicine

Background:

  • Physician-patient encounters can be challenging, with 1 in 6 labeled 'difficult'.
  • Difficult encounters disproportionately involve patients with medically unexplained or psychiatric conditions.
  • Younger, less experienced physicians report more difficult encounters.

Observation:

  • Two cases are presented: a male patient with irritable bowel syndrome post-surgery and a female patient with medically unexplained symptoms.
  • Neither patient had a treatable organic cause identified.
  • Both encounters were unsatisfactory for the patient and the physician.

Findings:

  • Difficult encounters negatively impact patient satisfaction.
  • Physicians experiencing frequent difficult encounters are at higher risk for burnout.
  • Lack of communication and psychosocial training contributes to difficult encounters.

Implications:

  • Improved communication skills training for physicians is crucial.
  • Integrating psychosocial medicine into medical education can mitigate difficult encounters.
  • Addressing the factors contributing to difficult encounters can enhance patient care and physician well-being.
