Background:

Purpose of the Study:

  • To investigate the incidence of suicide among individuals undergoing epilepsy surgery.
  • To examine the relationship between seizure control, depression, anxiety, and suicide post-surgery.

Main Methods:

  • A standardized protocol was used for patient history, diagnostic tests, and neuropsychiatric assessments.
  • The Beck Depression Inventory (BDI) and Beck Anxiety Inventory (BAI) were administered pre-surgically and annually for up to 5 years.
  • Mortality data, including cause of death, were collected for 396 enrolled patients.

Main Results:

  • Of 396 enrolled patients, 4 out of 27 deaths were attributed to suicide.
  • The standardized mortality ratio for suicide was 13.3 (95% CI=3.6-34.0) compared to the U.S. population, adjusted for age and gender.
  • Among the four patients who died by suicide, only one reported severe depressive symptoms (BDI=33), and two reported moderate to severe anxiety (BAI=17, 21).

Conclusions:

  • Suicide is a potential risk following epilepsy surgery, irrespective of reported seizure control.
  • Neuropsychiatric assessment, including depression and anxiety screening, is crucial for patients undergoing epilepsy surgery.
  • The findings underscore the importance of continued mental health surveillance in epilepsy patients post-intervention.
