Early fetal cystoscopy enables prenatal diagnosis and therapy for severe first-trimester megacystis. This minimally invasive procedure shows feasibility for improving outcomes in affected fetuses.
Area of Science:
- Perinatology
- Fetal Surgery
- Urology
Background:
- Severe first-trimester megacystis poses significant diagnostic and therapeutic challenges.
- Prenatal intervention for fetal uropathies is an evolving field.
Purpose of the Study:
- To assess the feasibility of early fetal cystoscopy for diagnosing and treating severe first-trimester megacystis.
- To evaluate outcomes following prenatal intervention.
Main Methods:
- Early fetal cystoscopy at 16 weeks gestation was offered to 15 patients with severe first-trimester megacystis.
- Six fetuses underwent the procedure; three had urethral atresia, and three had posterior urethral valves treated with laser fulguration.
- Infants were followed for 6-12 months postnatally.
Main Results:
- Urethral atresia was diagnosed in three fetuses, leading to pregnancy termination.
- Posterior urethral valves were successfully treated in three fetuses; two survived with normal renal and bladder function.
- Expectant management resulted in no survivals.
Conclusions:
- Percutaneous early fetal cystoscopy is a feasible technique for prenatal diagnosis and therapy of severe megacystis.
- Early intervention can improve survival and renal function in select cases.
