Cosmetic iris implants can cause severe vision problems, including optic nerve damage and cataracts. Early removal is often necessary to prevent permanent vision loss from elevated intraocular pressure (IOP).

Background:

  • Cosmetic iris implants, used since 2004 to alter eye color, involve implanting silicone diaphragms into the anterior chamber.
  • Limited data exists on the long-term side effects and potential complications associated with these cosmetic procedures.

Observation:

  • A 37-year-old patient developed significant ocular complications five months after cosmetic iris implant surgery.
  • The patient presented with optic nerve damage secondary to elevated intraocular pressure (IOP), nuclear opacity in both lenses, and a reduced number of corneal endothelial cells.

Findings:

  • Recurrent IOP elevation, despite maximal glaucoma therapy, necessitated the explantation of the iris implants.
  • Histopathological examination revealed damage to the trabecular meshwork, confirming a direct link to the implants.

Implications:

  • The observed ocular complications, including trabecular meshwork damage, lens opacity, and endothelial cell loss, are permanent.
  • These permanent changes increase the risk of future complications such as corneal decompensation and progressive glaucoma, highlighting the risks of cosmetic iris implantation.
