Summary

Microdrop-vitrification without cryoprotectants is a safe and effective method for preserving epididymal sperm obtained via percutaneous epididymal sperm aspiration (PESA). This technique offers a simple approach for cryopreserving low sperm numbers.

Background:

  • Epididymal sperm are crucial for treating male infertility.
  • Conventional cryopreservation methods can be challenging for limited sperm samples.
  • Percutaneous epididymal sperm aspiration (PESA) yields sperm suitable for cryopreservation.

Purpose of the Study:

  • To assess the feasibility and safety of cryopreserving epididymal sperm using microdrop-vitrification without cryoprotectants.
  • To compare microdrop-vitrification with conventional freezing for epididymal sperm.
  • To evaluate sperm revival rates, retrieval rates, and DNA integrity after cryopreservation.

Main Methods:

  • Epididymal sperm samples from 22 patients undergoing PESA were analyzed.
  • Samples were cryopreserved using conventional freezing (Group 1) or microdrop-vitrification without cryoprotectants (Group 2).
  • Revival rate, retrieval rate, and sperm nuclear DNA fragmentation were compared between groups.

Main Results:

  • While revival rates were similar (18.16% vs 21.99%), microdrop-vitrification showed a significantly higher retrieval rate (70.82% vs 58.39%, P < 0.01).
  • Pre-existing sperm nuclear DNA fragmentation (26.68%) did not increase after either freezing method.
  • Microdrop-vitrification proved effective for cryopreserving low sperm numbers without cryoprotectants.

Conclusions:

  • Microdrop-vitrification is a viable and safe method for cryopreserving epididymal sperm obtained via PESA.
  • This cryoprotectant-free technique is simple and effective for handling limited sperm samples.
  • The method preserves sperm quality and minimizes DNA damage during cryopreservation.
