Summary

Hydroxyurea (HU) can cause rare but serious leg ulcers in patients with myeloproliferative disorders. Discontinuing HU treatment is the most effective way to achieve complete healing of these ulcers.

  • Leg ulceration is a rare complication of HU therapy.

Observation:

  • A case study of a 70-year-old woman with polycythemia vera treated with HU.
  • The patient developed painful malleolar ulcers one year after starting HU.
  • Ulcers recurred after initial healing, becoming multiple and bilateral.

Findings:

  • Hydroxyurea-induced leg ulcers are rare and their pathogenesis is multifactorial.
  • Discontinuation of hydroxyurea treatment led to complete ulcer recovery in the reported case.
  • This highlights a significant, albeit uncommon, adverse effect of hydroxyurea.

Implications:

  • Clinicians should consider hydroxyurea-induced leg ulcers in patients presenting with such symptoms.
  • Discontinuation of HU should be strongly considered for managing these ulcers.
  • Further research into the pathogenesis of HU-induced ulcers is warranted.
