Summary

The new "slender system" with 0.010-inch guidewires and smaller catheters enables complex coronary interventions like bifurcation lesions and chronic total occlusions (CTO). This approach reduces complications and offers high success rates, challenging the need for larger devices.

Background:

  • Current coronary interventions commonly use 0.014-inch guidewires and 6Fr guiding catheters.
  • Larger guiding catheters (≥6Fr) are associated with increased punctured-site complications and radial artery occlusion, despite the benefits of the transradial approach.
  • Percutaneous coronary intervention (PCI) necessitates dual antiplatelet therapy, increasing bleeding risks with larger catheters.

Observation:

  • Development of a "slender system" in Japan, featuring 0.010-inch guidewires, 5Fr guiding catheters, and 3Fr angiography catheters.
  • This system is compatible with kissing balloon techniques (KBT) and has been utilized for complex lesions, including bifurcation lesions and chronic total occlusions (CTO).
  • The transradial approach was used in 90.7% of CTO cases treated with the slender system.
