Summary

Doripenem demonstrates strong activity against Gram-negative bacteria, outperforming imipenem and matching meropenem. It shows particular efficacy against Pseudomonas species, even in some cases of resistance to other carbapenems.

Background:

  • Doripenem is a novel carbapenem antibiotic recently introduced in Europe.
  • Serious Gram-negative bacterial infections pose a significant challenge in hospitalized patients.

Purpose of the Study:

  • To compare the in vitro activity of doripenem against imipenem and meropenem.
  • To evaluate the susceptibility of common Gram-negative pathogens to these carbapenems.

Main Methods:

  • The COMParative Activity of Carbapenem Testing (COMPACT) study involved 4498 Gram-negative isolates from 80 centers across Europe, the Middle East, and Africa.
  • Minimum Inhibitory Concentrations (MICs) were determined using Etest and broth microdilution methods.
  • Susceptibility was interpreted using CLSI, EUCAST, and FDA breakpoints.

Main Results:

  • Doripenem exhibited lower MIC(90) values than imipenem and meropenem for overall Gram-negative isolates (8 mg/L vs. ≥64 mg/L and 32 mg/L, respectively).
  • Doripenem was most potent against Pseudomonas species (MIC(90) of 16 mg/L) compared to imipenem and meropenem (≥64 mg/L).
  • Enterobacteriaceae were highly susceptible to all tested carbapenems, while Acinetobacter baumannii showed resistance.

Conclusions:

  • Doripenem displays excellent in vitro activity against Gram-negative bacteria, generally exceeding imipenem and matching meropenem.
  • Doripenem demonstrated superior activity against Pseudomonas species compared to both imipenem and meropenem.
  • A notable finding was doripenem's activity against some isolates resistant to imipenem and/or meropenem.
