Insights

The HLA-DRB1*1501 allele and TNF-alpha -308 G/A polymorphism increase multiple sclerosis risk in Iranians. Their combined presence significantly elevates susceptibility, highlighting gene interactions in MS pathogenesis.

Area of Science:

  • Immunogenetics
  • Neuroimmunology

Background:

  • Multiple sclerosis (MS) is a complex, multifactorial neurological disorder.
  • Genetic factors, including Human Leukocyte Antigen (HLA) and cytokine genes, are implicated in MS pathogenesis.

Purpose of the Study:

  • To investigate the association of the HLA-DRB1*1501 allele and the TNF-alpha -308 G/A single nucleotide polymorphism (SNP) with MS susceptibility in the Iranian population.
  • To evaluate the potential interaction between these genetic markers in MS risk.

Main Methods:

  • Genomic DNA was extracted from 366 MS patients and 414 healthy controls.
  • Genotyping for HLA-DRB1*1501 and TNF-alpha -308 G/A polymorphism was performed using SSP-PCR.
  • Allele and genotype frequencies were compared using Fisher's exact test.

Main Results:

  • The HLA-DRB1*1501 allele was significantly more frequent in MS patients (OR=1.57, P=0.0026).
  • The TNF-alpha -308 G allele and G/G genotype were also associated with increased MS risk (G vs. A: OR=1.26, P<0.05; G/G vs. A/A: OR=4.59, P=0.0003).
  • A significant interaction was observed, with the co-existence of TNF G and HLA-DRB1*1501 alleles showing a higher prevalence in MS patients (OR=7.07, P=0.0007).

Conclusions:

  • Both HLA-DRB1*1501 and TNF-alpha -308 G/A polymorphism are risk factors for MS in the Iranian population.
  • Evidence supports a gene-gene interaction between HLA and cytokine genes in the development and pathogenesis of multiple sclerosis.

Pharmacogenetics of Drug Targets: β₂-Adrenergic Receptors, Apo E, Thymidylate Synthase

Genetic polymorphisms in drug targets have emerged as critical determinants of interindividual variability in drug response and toxicity. Pharmacogenomic investigations increasingly focus on identifying these variations to personalize and optimize therapeutic interventions. A drug target may be a receptor, enzyme, or signaling protein involved in pharmacologic responses or disease-related pathways. While early pharmacogenetic studies focused primarily on drug metabolism, current research...

Pharmacogenetic Phenotypes: Alterations in Pharmacokinetics, Drug Targets and Biologic Milieu

Genetic variations significantly influence drug response through pharmacokinetics, receptor interactions, and biologic milieu modifications. Pharmacokinetic alterations impact drug metabolism and clearance, affecting efficacy and toxicity. Variants in drug-metabolizing enzymes, such as CYP2C9 and CYP2C19, alter drug activation and elimination. For example, CYP2C9 loss-of-function variants require lower warfarin doses to prevent excessive bleeding, while CYP2C19 variants reduce clopidogrel...

Genome-wide Association Studies-GWAS

Genome-wide association studies or GWAS are used to identify whether common SNPs are associated with certain diseases. Suppose specific SNPs are more frequently observed in individuals with a particular disease than those without the disease. In that case, those SNPs are said to be associated with the disease. Chi-square analysis is performed to check the probability of the allele likely to be associated with the disease.
GWAS does not require the identification of the target gene involved in...
T Cell Types and Functions01:24

T Cell Types and Functions

When T cells with CD4 markers are activated, they give rise to two types of effector cells: helper T cells and regulatory T cells. Meanwhile, T cells with CD8 markers differentiate into effector cytotoxic T cells. The differentiation of CD4 T cells into helper T cell subsets, such as Th1, Th2, and Th17 cells, is dependent on the antigen type, antigen-presenting cell, and regulatory cytokines.
Th1 cells stimulate dendritic cells to express necessary co-stimulatory molecules on their surfaces for...

Multiple Sclerosis l: Introduction

Multiple sclerosis is a chronic autoimmune disease of the central nervous system (CNS) that affects the brain, spinal cord, and optic nerves. It is an inflammatory demyelinating disorder and a leading cause of neurological disability in young adults.EpidemiologyMS commonly begins between 20 and 40 years of age and is twice as common in women. Its exact cause remains unclear, but genetic susceptibility contributes, with higher risk in first-degree relatives and identical twins. A greater...
