Summary

Poststroke depression (PSD) severity at baseline is linked to functional dependence after stroke. While improvement in PSD may not directly impact recovery, treating depression is crucial for patient well-being.

  • Analysis conducted on the total cohort (367 participants) and the PSD subgroup (174 participants).
  • Main Results:

    • In the PSD cohort, increased medical comorbidity, stroke severity, and baseline depression severity were independently associated with 12-week functional dependence.
    • Depression severity (PHQ-9 scores) was significantly higher in dependent individuals compared to independent individuals at 12 weeks in both the entire cohort and the PSD subgroup.
    • Odds ratios indicated significant associations: medical comorbidity (OR 1.10), stroke severity (OR 1.42), and baseline depression severity (OR 1.13).

    Conclusions:

    • Baseline depression severity is associated with functional dependence 12 weeks poststroke.
    • The study results are inconclusive regarding whether depression improvement independently influences functional recovery.
    • Identifying and treating PSD is vital due to its symptom burden and association with adverse health and social outcomes, irrespective of direct impact on functional recovery.
