Background:

  • Malformative uropathies are a significant cause of end-stage renal disease (ESRD) necessitating renal replacement therapy (RRT).
  • Advances in medical and surgical care have improved outcomes for kidney transplantation in this patient group.

Purpose of the Study:

  • To describe the epidemiological profiles of patients with ESRD due to malformative uropathies who underwent kidney transplantation.
  • To evaluate and compare the outcomes of kidney transplantation in these patients versus those without malformative uropathies.

Main Methods:

  • Retrospective study of 47 kidney transplant recipients with malformative uropathies (1986-2009).
  • Analysis of acute rejection episodes, acute tubular necrosis, patient survival, and graft survival.
  • Comparison with control group using chi-square and Student t tests; survival curves analyzed with log-rank tests.

Main Results:

  • Malformative uropathies accounted for 9.53% of kidney transplants; vesico-ureteral reflux was the common etiology.
  • Lower incidence of acute tubular necrosis (8.5% vs 22.06%, P=.03) in the malformative uropathy group.
  • Comparable patient and graft survival rates at 1, 5, and 10 years between groups (P=NS).

Conclusions:

  • Kidney transplantation for malformative uropathies is becoming more common.
  • Outcomes, including acute rejection rates and survival, are similar to patients without malformative uropathies.
  • Successful kidney transplant results support transplantation as a viable option for ESRD caused by uropathies.
