    Summary

Orchidopexy improves fertility for some boys with undescended testes. However, about 65% of bilateral cases remain sterile, and unilateral cases show impaired fertility, impacting paternity hopes.

    Area of Science:

    • Pediatric Surgery
    • Urology
    • Reproductive Medicine

    Background:

    • Undescended testes (cryptorchidism) affect numerous children.
    • Orchidopexy is a common surgical treatment for cryptorchidism.
    • Assessing fertility outcomes post-orchidopexy is crucial for patient management.

    Purpose of the Study:

    • To appraise the fertility outcomes following orchidopexy in children with undescended testes.
    • To evaluate the effectiveness of orchidopexy for cosmetic and psychological benefits.
    • To determine the spermatogenesis capacity after surgical correction.

    Main Methods:

    • Retrospective analysis of 567 unilateral and 202 bilateral cases treated since 1956.
    • Fertility assessment primarily via semen analysis in 76 unilateral and 29 bilateral cases (average age 19).
    • Anatomical assessment and testicular biopsy in select cases to correlate with spermatogenesis.

    Main Results:

    • Approximately 10% of bilateral undescended testes achieve normal spermatogenesis post-orchidopexy; 65% remain sterile.
    • Unilateral undescended testes show impaired fertility: 28% have reduced paternity potential, 4% are infertile.
    • Anatomical appraisal and biopsy provided limited additional information on spermatogenesis compared to overall assessment.

    Conclusions:

    • Orchidopexy offers some fertility improvement but does not guarantee restoration in all cases, particularly bilateral cryptorchidism.
    • Significant rates of infertility persist after treatment, highlighting the need for long-term monitoring.
    • The study underscores the importance of assessing functional fertility outcomes beyond cosmetic results of orchidopexy.
