Insights

Sunitinib, a vascular endothelial growth factor receptor (VEGFR) inhibitor, showed no objective responses in patients with relapsed diffuse large B-cell lymphoma (DLBCL). The trial was stopped early due to lack of efficacy and significant hematologic toxicity.

Area of Science:

  • Oncology
  • Pharmacology

Background:

  • Limited effective therapies exist for relapsed or refractory diffuse large B-cell lymphoma (DLBCL).
  • Vascular Endothelial Growth Factor Receptor (VEGFR) kinase inhibitors have shown activity in solid tumors.

Purpose of the Study:

  • To evaluate the efficacy and safety of sunitinib in adult patients with relapsed or refractory DLBCL.

Main Methods:

  • Phase II clinical trial involving 19 patients with relapsed/refractory DLBCL.
  • Sunitinib administered orally at 37.5 mg once daily.
  • 17 patients were evaluable for toxicity, 15 for response.

Main Results:

  • No objective responses were observed.
  • Nine patients (60%) achieved stable disease with a median duration of 3.4 months.
  • Grades 3-4 neutropenia (29%) and thrombocytopenia (35%) were significant toxicities.
  • No correlation found between circulating endothelial cell (CEC) counts and tumor burden.

Conclusions:

  • Sunitinib demonstrated no efficacy in relapsed/refractory DLBCL.
  • The drug exhibited higher-than-expected hematologic toxicity in this patient population.
  • The study was discontinued early due to lack of therapeutic benefit.
