Complex sleep apnoea (CompSA) is not always persistent and often occurs in predisposed individuals during nights with poor sleep quality. CompSA development and resolution vary over time in patients undergoing continuous positive airway pressure (CPAP) treatment.
Area of Science:
- Sleep Medicine
- Respiratory Medicine
Background:
- Complex sleep apnoea (CompSA) can emerge during continuous positive airway pressure (CPAP) therapy for obstructive sleep apnoea (OSA).
- The stability and predictors of CompSA over time require further investigation.
Purpose of the Study:
- To investigate the incidence, persistence, and resolution of CompSA in patients treated with CPAP.
- To identify patient characteristics and sleep parameters associated with CompSA.
Main Methods:
- Prospective study involving 675 OSA patients undergoing polysomnography at diagnosis, first CPAP night, and 3-month follow-up.
- Assessment of CompSA, sleep quality, and sleepiness using polysomnography and standardized measures.
Main Results:
- 12.2% of patients had initial CompSA; only 26% of these persisted at follow-up.
- 16 patients (4.2%) developed new-onset CompSA after 3 months of CPAP.
- CompSA patients were older, had higher rates of coronary artery disease, and experienced more wakefulness after sleep onset.
Conclusions:
- CompSA is dynamic, with significant rates of both resolution and new onset.
- CompSA is associated with specific patient predispositions and impaired sleep quality during CPAP treatment.
- Further research is needed to elucidate the causal relationship between sleep impairment and CompSA.
