Summary

A rare primary synovial sarcoma of the pericardium was treated with surgery, chemotherapy, and tomatherapy. Cardiac MRI guided follow-up, resulting in a patient survival of 26 months, exceeding previous reports.

Background:

Primary pericardial tumors are exceptionally rare.
Synovial sarcoma is a soft tissue malignancy typically arising in extremities.
Pericardial synovial sarcoma represents an extremely rare clinical entity.

Observation:

A case of primary synovial sarcoma originating in the pericardium is presented.
The patient underwent incomplete surgical resection.
Adjuvant therapies including chemotherapy and intensity-modulated radiotherapy (tomotherapy) were administered post-operatively.

Findings:

Cardiac magnetic resonance imaging (MRI) was instrumental in monitoring treatment response and planning adjuvant therapies.
The patient achieved a survival of 26 months.
This survival duration is notably longer than typically reported for this rare condition.

Implications:

This case highlights the potential for aggressive management of rare pericardial malignancies.
Cardiac MRI is a valuable tool for surveillance and therapeutic planning in pericardial tumors.
Extended survival may be achievable with multimodal treatment strategies for primary pericardial synovial sarcoma.
