Context:

Bladder cancer diagnosis and monitoring rely on cytology and cystoscopy.
Cytology has variable sensitivity; cystoscopy is invasive and has limitations.
A sensitive and specific urinary test could offer an improved alternative.

Purpose:

To evaluate the diagnostic value of Nuclear Matrix Protein 22 (NMP22) as a replacement for cytology and cystoscopy in bladder cancer management.
To assess NMP22's sensitivity and predictive values in screening and follow-up contexts.

Summary:

A literature review identified 17 studies on NMP22 for bladder cancer (1996-2010).
NMP22 demonstrated higher sensitivity than cytology for screening and follow-up.
NMP22 showed a positive predictive value of 0-70% for screening and ~70% for follow-up, reaching up to 90% when combined with cytology.

Impact:

NMP22 is recommended as a reference test for specific screening cases and for ruling out false positives.
The combination of NMP22 and cytology is proposed as the new reference for follow-up.
Positive NMP22 with negative cystoscopy warrants careful follow-up due to increased recurrence risk.
